Vaccination Record Form

A vaccination record form or immunization record is a document which is provided by a hospital or clinic to an individual to keep track of their vaccinations. An individual’s vaccination record form is considered very important. It is usually prepared for a person when they are an infant and get their first mandatory vaccination doses. This record form keeps all information about all the different vaccines the susceptible person has received in their lifetime and the ones they have not received yet. These forms even inform you about how many doses of each vaccine the person has received and how many are pending. Hence, it is considered an important document for any individual, just like their birth certificate.

Why a Vaccination Record Form is Used?

When we apply to any organization as an employee or get admission to an institute or school, the administration demands some documents for our identification. One of these documents is a vaccination record form. And the reason is quite clear, as we are well aware that some viral diseases are contagious and need to be contained. They can be transmitted by touch if the person is infected. Hence, for the safety of the other people present in the facility, it is important for everyone to present their vaccination forms to the authorities of that facility. This way, they have an idea of how prone the individual is to most viral diseases. In some companies/schools it is important for the candidate to be vaccinated, or else they are rejected from the position. In other cases, they are only asked to present their immunization record.

Importance of a Vaccination Record Form:

Keeping track of your or your child’s vaccination record can be a tough job because it is something that can slip out of our minds because of our busy schedule. However, when we have a vaccination record of our vaccines, we can look at the table and see which shot we need to get next and when. It also marks out the diseases whose vaccination you have received, so you don’t have to worry about becoming a victim to that disease. Additionally, now that there are vaccines for almost all viral diseases, it is appreciated to get yourself and your children vaccinated because vaccination records are demanded in all facilities, including schools.

What does a Vaccination Record Form look like?

A vaccination record template is a document which is generally made on an Excel sheet. At the very top of the document there is the name of the hospital or faculty which issued this record. Below it, there is personal information about the individual such as their name, gender, date of birth and address. Mainly, it has multiple rows and columns, in which every row has the name of different viral diseases such as hepatitis B, chicken pox and mumps. Whereas, the columns have different headings, such as dose 1, dose 2, the date on which the vaccine was received, number of pending doses and signature of the vaccinated person. These headings can vary depending on the hospital from which you get your vaccination record.
