An affidavit form is a document that has legal importance which is generally required for various reasons, such as buying and selling, applying for visas, as well as admissions to schools or colleges. 3- This affidavit form is also used to identify the true identity of an individual, in a case where it is compromised.
4- This form affirms the actual and present residential address of an individual.
5- It is used to inform a third party about the demise of a person in the absence of a certificate of death.
6- When an individual has changed his/her name and want to verify it. He/she needs to present his/her birth certificate along with this form.
